I have understood what you are asking already. Did you apply the new version? I have sent the download link to your email address.

The problem you are having is a Search Engine Friendly URL issue. If you disable the SEF URL creation from joomla configuration; you will notice it loads only the form (instead of whole webpage).
But it should be fixed with the new version. It should show only form even though the SEF URL is enabled.
