hmm, that's weird. Category title and grid-list options have nothing to do with module. They are being printed inside the "content" area (content-body)

I believe the elements are still there, but they got moved to top, under the menu-nav bar so you won't see them (because there is no element to clear the floating and separate the main-menu and content-body).

FYI: in your 2nd screenshot, i am seeing a 1px line under the "CONTACT" menu item, which must be the bottom line of the GRID-LIST changer.

Provide me your website URL where this happens and let me see the sources. I think, this can be fixed with a simple CSS patch.
