Our premium support service is based on the time we spend on your website. We charge $59 per hour. So the cost is really depends on how complicated things you are requesting. Once we take care the modifications, you will get a report that tells what settings/parameters we have edited in the joomla backend, and what PHP/CSS files we have modified. This will give you an idea about what to do. If you have basic coding/joomla knowledge; then you will probably learn "how to do" and then you can then apply further modifications by yourself.

Some of the modifications you need will require a lot of effort, most of them are not technical at all; they are just time consuming things (especially #2, #4 and #5). You can use 3rd party plugin/components to achieve them. You don't need the premium support service from us; it will be too expensive approach for you. For instance: for #5, use zopim.com

If you want me to give you a quote for them though, roughly;
#1 -> 1 hour
#2 -> 1/2 hour
#3 -> ~2 hours (i actually don't understand what you meant by this; you should give me more details about it)
